An Automated Process for Batch Preparation of Epidural Solutions Containing Bupivacaine Hydrochloride and Fentanyl Citrate

Abstract:  Epidural solutions containing combinations of bupivacaine hydrochloride and fentanyl citrate are commonly administered in the labor and delivery and postoperative settings for pain control. The author describes the method used at his institution, Millard Fillmore Suburban Hospital in Williamsville, NY, for batch preparing epidural solutions for its large base of labor patients. He describes the method used, which involves use of a foot-activated, volume-repeating pump and closed-system transfers. He reports that the compounding of epidural solutions using the automated process described has been used for over 4 years, with more than 300 batches completed. Anesthesiologists are pleased with the program, and patients receive their epidurals more quickly than if the anesthesiologists were to prepare the solution. A discussion concludes the article.
